• Information for COVID Surveillance Testing

    As part of our plan for returning to all-in person learning, we are offering free non-invasive nasal swab COVID-19 testing to students attending Hastings-on-Hudson Schools. The program is voluntary and requires parental consent using this form. There will be weekly testing of 5% of the student body in each school using the BinaxNow rapid test. The consent form contains a more detailed explanation of the testing program.

    To ensure the testing program operates smoothly, please follow the steps outlined below. 

    • Hillside Elementary School testing will take place in the Nurse’s Office on Mondays from 3:15pm-4:15pm. Elementary children must be accompanied by a parent during testing. Please complete this Pre-Test Registration Form no later than 1:00pm on the day of the test. 
    • Farragut Middle School testing will take place in the Nurse’s Office on Mondays from 3:15pm-4:15pm. Parental accompaniment is not required. Please complete this Pre-Test Registration Form no later than 1:00pm on the day of the test.
    • Hastings High School testing will take place in the Nurse’s Office on Tuesdays from 3:15pm-4:15pm. Parental accompaniment is not required. Please complete this Pre-Test Registration Form no later than 1:00pm on the day of the test.
    • The BinaxNow test uses a nasal swab with results available in 15 minutes. No one will be allowed to wait for the results of the test. 
    • Anyone who tests positive will receive a phone call with instructions and guidance from your child’s School Nurse. 
    • Anyone who tests negative will receive an email and no further action is needed.
    • We are required to report the results of each test to the New York State Department of Health.

    Forms to Complete

    1. Please complete the Pre-Test Registration Form no later than 1:oopm on the day of the test.
    2. Please print and complete the Parental Consent Form and bring it or send it with your child to the Nurse’s Office.
    3. Please print and complete the top portion of the COVID Results Form and bring it with you on the day of the test.
