Instructions
Who may use this application for a school district early mail ballot?
You may use this application if you are a qualified voter participating in a school election for which early vote by mail is authorized by law. You may only apply for an early mail ballot on your own behalf. A voter who applies for and is issued an early mail ballot will not be eligible for an absentee ballot for the same election.
If you are unsure whether the election you are applying for permits early vote by mail, please contact your district clerk.
Who is a qualified voter?
You are qualified to vote in your school district if you are:
• a citizen of the United States;
•at least 18 years of age; and
• a resident of the school district for a period of at least 30 days preceding the meeting or election at which you seek to vote.
No person shall have the right to register for or vote at any school meeting or election who would not be qualified to register for or vote at an election in accordance with the provisions of Election Law §5-106.
Information for military voters:
Do not use this application if you are:
• a qualified voter who will be absent from your school district on the day of the election as a result of actual military service;
• a qualified voter who has been discharged from actual military service within 30 days of the election in which you seek to vote; or
• the spouse, parent, child, or dependent of a military voter as set forth above who is accompanying such military voter and who is qualified to vote in the same school district as the military voter.
If you meet any of the above criteria, you are entitled to special provisions if you apply for a military ballot. Please contact your school district to receive the appropriate application form.
Information for voters with an illness or disability:
You may sign the early mail ballot application yourself, or you may make your mark and have your mark witnessed in the spaces provided on the bottom of the application. Please note that a power of attorney or printed name stamp is not allowed for any voting purpose.
Where and when to return this application:
If you request that the early mail ballot be mailed to you, your application must be received by the district clerk for your school district no later than 7 days before the election (May 12, 2026) for which you seek an early mail ballot. Otherwise, you may personally deliver your application to the district clerk no later than the day before the election (May 18, 2026). You may not submit your application more than 30 days prior to the election (April 20, 2026).
When your early mail ballot will be sent to you:
If you request that the early mail ballot be mailed to you, the district clerk will mail your ballot by regular mail no later than 6 days prior to the election (May 13, 2026). Otherwise, the district clerk will deliver your ballot to you or your agent, as designated on your application, when you or your agent appears in the district clerk’s office. For your ballot to be canvassed, it must be received by the school district clerk by 5 p.m. on the day of the election.

¿Quién puede utilizar esta solicitud para una boleta de votación anticipada por correo del distrito escolar?
Puede utilizar esta solicitud si es un votante calificado que participa en una elección escolar para la cual la ley autoriza el voto anticipado por correo. Sólo puede solicitar una boleta anticipada por correo en su propio nombre. Un votante que solicita y recibe una boleta de voto anticipado por correo no será elegible para una boleta de voto ausente para la misma elección.
Si no está seguro de si la elección para la que está solicitando permite el voto anticipado por correo, comuníquese con el secretario de su distrito.
¿Quién es un votante calificado?
Usted está calificado para votar en su distrito escolar si:
• un ciudadano de los Estados Unidos;
•tener al menos 18 años de edad; y
• un residente del distrito escolar durante un período de al menos 30 días antes de la reunión o elección en la que desea votar.
Ninguna persona tendrá derecho a registrarse o votar en cualquier reunión o elección escolar que no esté calificada para registrarse o votar en una elección de acuerdo con las disposiciones de la Ley Electoral §5-106.
Información para votantes militares:
Hacer no utiliza esta aplicación si eres:
• un votante calificado que estará ausente de su distrito escolar el día de la elección como resultado del servicio militar real;
• un votante calificado que haya sido dado de baja del servicio militar real dentro de los 30 días posteriores a la elección en la que desea votar; o
• el cónyuge, padre, hijo o dependiente de un votante militar según lo establecido anteriormente que acompaña a dicho votante militar y que está calificado para votar en el mismo distrito escolar que el votante militar.
Si cumple alguno de los criterios anteriores, tiene derecho a disposiciones especiales si solicita una boleta militar. Comuníquese con su distrito escolar para recibir el formulario de solicitud correspondiente.
Información para votantes con enfermedad o discapacidad:
Usted puede firmar la solicitud de boleta anticipada por correo usted mismo, o puede dejar su marca y hacer que su marca sea atestiguada en los espacios provistos en la parte inferior de la solicitud. Tenga en cuenta que no se permite un poder notarial ni un sello con el nombre impreso para ningún propósito de votación.
Dónde y cuándo devolver esta solicitud:
Si solicita que le envíen la boleta de votación anticipada por correo, el secretario del distrito escolar debe recibir su solicitud a más tardar 7 días antes de la elección (12 de mayo de 2026) para la cual solicita una boleta de votación anticipada por correo. De lo contrario, podrá entregar personalmente su solicitud al secretario del distrito a más tardar el día anterior a la elección (18 de mayo de 2026). No podrá presentar su solicitud más de 30 días antes de la elección (20 de abril de 2026).
Cuándo se le enviará su boleta de votación anticipada por correo:
Si solicita que se le envíe por correo la boleta de votación anticipada, el secretario del distrito enviará su boleta por correo postal regular a más tardar 6 días antes de la elección. (13 de mayo de 2026) De lo contrario, el secretario del distrito le entregará su boleta a usted o a su agente, según lo designado en su solicitud, cuando usted o su agente se presenten en la oficina del secretario del distrito. Para que su boleta sea escrutada, el secretario del distrito escolar debe recibirla antes de las 5 p. m. del día de la elección.
